FollowMyHealth Patient Portal

Northwell Health is pleased to offer FollowMyHealth®, a patient portal that allows you to manage your personal health information—anywhere, anytime. You can review a summary of your doctor’s visit or review your discharge instructions after a hospital stay. With FollowMyHealth, you will be able to take a more active role in managing your health and wellness with your Northwell Health health care team.

You can create a free portal account using your computer, smartphone or tablet to access your personal health information. There are a number of features available, and new features like online bill pay and making or changing appointments are coming soon. Go to Northwell.edu for more information.

• View lab and test results after a hospital stay or doctor’s visit.
• Request medication renewals from participating doctors' offices.
• Access your health profile to view your care plan, allergies and medication information.
• Communicate with your doctor’s office through secure messaging (now available at select practices).

Download the free portal app at your Google Play or iTunes Store. Enter “FollowMyHealth” in the search field.

Get Started

From an email invitation:

1. Check your email for an invitation from Northwell Health. Click the registration link and follow the on-screen steps to create your account.
2. Click "Create an Account" or, if you already have a FollowMyHealth account, click "Log In."
3. Choose one of the Authentication Methods (e.g., Facebook, Google) provided or create a FollowMyHealth™ username and password. FollowMyHealth will never share your personal health information with the Authentication Method you choose.
4. Follow the on-screen steps to complete your registration. This will include your acceptance of the FollowMyHealth Terms of Use.

You will receive an email confirming your account has been created and a “Welcome” email from FollowMyHealth. You now have access to your health information!

Self-Registration at Northwell.followmyhealth.com:

1. Click "Create an Account" or, if you already have a FollowMyHealth account, click "Log In."
2. Choose one of the Authentication Methods (e.g., Facebook, Google) provided or create a FollowMyHealth™ username and password. FollowMyHealth will never share your personal health information with the Authentication Method you choose.
3. Create your account—please enter as much information as you are able, including the optional fields. This will include your acceptance of the FollowMyHealth Terms of Use.
4. Enter your zip code in the search field and find Northwell Health. Click "Connect."
